In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

In addition the facility failed to develop policies and procedures related to the monthly drug regimen review. The findings were: Review of the pharmacist's medication regimen reviews for resident #11 showed the monthly reviews were being completed, however there was no evidence a separate written report listing irregularities or recommendations was submitted to the physician. Interview with the pharmacist on 3/27/19 at 8:31 AM revealed he had developed a system on his personal computer for completing the monthly reviews and would then transfer the information into the resident's medical record. However, he did not generate a separate report with any irregularities or recommendations to the required facility personnel. Interview with the DON on 3/27/19 at 9:14 AM verified the required information was not in the medical record. In an additional interview on 3/27/19 at 2:44 PM the DON stated a policy and procedure for the monthly medication review had not been developed. 2020-09-01
