In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The census was 29. The findings were: 1. Observation of the facility during the survey timeframe showed Resident/Family Grievance forms were located next to the information board in the main area of the facility. However, the grievance policy and contact information for the grievance official were not posted. 2. Review of the policy and procedure entitled Resident Grievance Policy last revised 4/25/16 showed the policy failed to contain required information including the contact information of the grievance official and the contact information of independent entities with whom a grievance could also be filed. Further, the policy did not address the residents' right to file a grievance anonymously. 3. Interview with the social service director on 3/26/19 at 5 PM verified the policy did not contain the required information and the residents had not been informed individually or through postings.	2020-09-01
