In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The findings were: 1. Review of the medical record for discharged resident #29 on 3/13/19 10:22 AM showed that a comprehensive discharge plan had not been developed. An Interdisciplinary Discharge Summary that was used as documentation was not complete, and did not include elements necessary to prevent unnecessary readmissions such as a reconciliation of pre and post discharge medications. There was no indication of where the resident planned to live, nor was there documentation that pertinent labs, prescriptions, or new home medications had been forwarded to the resident's primary care provider. 2. In an interview on 3/13/19 04:45 PM, the administrator and medical records staff #1 confirmed that the Interdisciplinary Discharge Summary was the only documentation in use and acknowledged it did not meet all of the requirements. 2020-09-01
