In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The findings were: 1. Observation on 4/7/19 at 4:35 PM of the Victory refrigerator in the main kitchen and again with the certified dietary manager on 4/9/19 at 4:05 PM showed 7 open cartons of pre-thickened beverages and 2 open containers of thickened dairy drink were not labeled with an open date. 2. Observation on 4/7/19 at 6:22 PM of the refrigerator located on the first floor showed 2 open cartons of pre-thickened liquid containers and 1 open container of thickened dairy drink were not labeled with an open date. 3. Observation on 4/10/19 at 4:55 PM of the refrigerator located in the Reflections unit showed 2 open cartons of thickened dairy drink were not labeled with an open date. 4. Observation on 4/10/19 at 5 PM of the refrigerator located on the third floor showed 1 open carton of thickened dairy drink was not labeled with an open date. 5. Interview on 4/9/19 at 4:05 PM with the certified dietary manager confirmed the open cartons of thickened beverages were not labeled with an open date. 6. Review of the product label of the pre-thickened beverages showed the product was good for 7 days after opening if refrigerated. In addition, review of the manufacturer's instructions for the thickened dairy drink showed once opened, recap, refrigerate and use within 48 hours. 7. According to Food Code (YEAR), U.S. Public Health Service: 3-501.17 (A) .refrigerated, READY-TO-EAT, POTENTIALLY HAZARDOUS FOOD (TIME/TEMPERATURE CONTROL FOR SAFETY FOOD) prepared and held in a FOOD ESTABLISHMENT for more than 24 hours shall be clearly marked to indicate the date or day by which the FOOD shall be consumed on the PREMISES, sold, or discarded when held at a temperature of 5 C (41 F) or less for a maximum of 7 days. 2020-09-01
