In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Based on record review, staff interview and Minimum Data Set (MDS) Resident Assessment Instrument (RAI) version 3.0 manual, the facility failed to ensure the accuracy of a MDS for a resident receiving Hospice services. This practice was found for one (1) of one (1) residents reviewed for Hospice services. Resident identifier: #130. Facility census: 140. Findings included: a) Resident #130 On 07/31/19 at 11:45 AM, review of the medical record revealed Resident #130 was admitted to Hospice services on 07/19/19. Review of the significant change MDS with assessment reference date (ARD) of 07/21/19 discovered the following: Section J, titled Health Conditions, J1400 Prognosis, coded as: NO. MDS RAI version 3.0 manual coding instructions for J1400 Prognosis (typed as written): . --Code 1, yes: if the medical record includes physician documentation: 1) that the resident is terminally ill; or 2) the resident is receiving hospice services. After review of the significant change MDS with ARD of 07/21/19 on 07/31/19 at 11:55, Employee #152 stated, Yes, the manual states it should be coded yes because the resident is on Hospice. But the coordinator was waiting on physician documentation. Yes, if you follow the manual which they are supposed to, it is coded wrong. |
