In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: Per record review, Record #1 has a [DIAGNOSES REDACTED]. During observation on 10/9/17 and 10/10/17, Resident #1 was up in a wheelchair with a harness that covered his/her chest. The harness had two straps; the straps went over the resident's shoulders and then clipped in the back of the wheelchair, securing the resident in an upright position. The medical record did not have a physician's orders [REDACTED]. The Unit Manager and the Administrator confirmed this on 10/9/17 at 11:57 PM. Resident #1's care plan for falls read, (MONTH) use chair harness with staff and/or family supervision. There was no indication for use of the chair harness and what monitoring was to be done while the harness was on Resident # 1.
