In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Specifically, a resident did not receive oral hygiene for two days. Resident identifier: 32. Findings include. Resident 32 was admitted to the facility on [DATE] with [DIAGNOSES REDACTED]. On 6/18/18 at 12:29 PM, an interview was conducted with resident 32. Resident 32 was observed to have food debris in her mouth that was light pink in color. There was a hand-written sign taped on the wall with care information documented. The hand-written sign instructed staff to, .Help her brush teeth daily. On 6/19/18 at 8:15 AM, resident 32 was observed. Resident 32 was observed to have light pink in color food debris in her mouth. Resident 32's medical record was reviewed on 6/20/18. Review of the Activities of Daily Living care plan that was developed for resident 32 on 2/2/18 and revised on 4/12/18 revealed that the facility staff were to provide extensive assistance of one staff member for oral care. On 6/19/18 at 12:56 PM, an interview was conducted with Certified Nurse Assistant (CNA) 6. CNA 6 stated that resident 32 needed assistance with personal hygiene including oral care. CNA 6 stated that she did not assist resident 32 with brushing her teeth on 6/18/18 or 6/19/18. On 6/19/18 at 1:05 PM, an interview was conducted with Licensed Practical Nurse (LPN) 3. LPN 3 stated that resident 32 was to have her teeth brushed twice a day. On 6/19/18 at 1:20 PM, an interview was conducted with CNA 8. CNA 8 stated that she did not assist resident 32 with brushing her teeth. 2020-09-01
