In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Specifically, results of a basic metabolic panel (BMP) blood test were not reported to the physician. Resident identifier: 16. Findings include: Resident 16 was admitted to the facility on [DATE] with [DIAGNOSES REDACTED]. On 5/23/19, a medical record review was conducted for resident 16. A review of a nursing note dated 2/16/19 at 2:04 PM, revealed, Labs: BMP collected and MD (Medical Doctor) reviewed all lab values, New Orders; repeat BMP X (times) 1 week. Results for a BMP obtained on 2/20/19 were faxed to the facility on [DATE] at 7:35 AM. On 5/23/19 at 7:15 AM, an interview was conducted with Registered Nurse (RN) 3. RN 3 stated that nurses signed lab reports when they received them and then nurses contacted the physician. RN 3 stated that the faxed test results were received this morning, and were not received previously. RN 3 stated that he signed the order today. On 5/23/19 at 10:15 AM, an interview was conducted with the Director of Nursing (DON). The DON stated that there was no record that nursing staff had received the results or contacted the physician about the results from the lab test on 2/20/19. 2020-09-01
