In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Specifically, a resident on hospice did not have visits from hospice staff available in the medical record. Resident identifier: 56. Findings include: 1. Resident 56 was admitted to the facility on [DATE] with [DIAGNOSES REDACTED]. On 4/23/18 resident 56's medical record was reviewed. It was noted that the last hospice visit documented in the medical record was from 3/20/18. In the resident's progress notes the hospice RN had charted on 3/28, 4/10, 4/23, 4/26 and 4/30. On 4/26/18 at 9:40 AM an interview was conducted with the RN Case Manager of the hospice company. The RN Case Manager stated that she had notes with her from about 4/12/18 to current that needed to be given to the facility. On 4/30/18 at 2:50 PM an interview was conducted with the Medical Records Director (MRD). The MRD stated that the hospice staff was supposed to send over copies of their charting. The MRD stated that the facility would like to receive hospice notes within a week. Requested the MRD to search resident 56's record for notes beyond 3/20/18 from the hospice staff. The MRD was unable to find further documentation. The MRD acknowledged there was an incomplete medical record. 2020-09-01
