In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Specifically resident's MDS and careplan contained conflicting information. Resident identifiers 7 and 18. Findings include: 1. Resident 18 was admitted to the facility on [DATE] with [DIAGNOSES REDACTED]. On 4/25/18, Resident 18's medical record was reviewed. Resident 18's MDS section G0110 1B revealed that resident 18 required a 2+ person extensive assistance for transfers. Resident 18's care plan revealed that resident 18 required 1-2 extensive assistance for ADL's. 2. Resident 7 was admitted to the facility on [DATE] with [DIAGNOSES REDACTED]. On 4/25/18, Resident 18's medical record was reviewed. Resident 7's MDS section G0110 1B revealed that resident 7 required a 2+ person extensive assistance for transfers.
