In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on surveyor observation, record review and staff interview, it has been determined the facility failed to assure that services being provided meet professional standards of quality relative to following physicians MEDICATION ORDERS FOR [REDACTED]. Findings are as follows: Mosby's 4th Edition, Fundamentals of Nursing, page 314 states, The physician is responsible for directing medical treatment. Nurses are obligated to follow physician's orders [REDACTED]. Clinical record review for resident ID#71 reveals a physician's orders [REDACTED]. Further record review of the Medication Administration Record [REDACTED] The 7:30 AM dose was administered late on 14 out of 59 dates; 05/16/2018, 05/20/2018, 05/21/2018, 05/22/2018, 05/24/2018, 05/30/2018, 05/31/2018, 06/13/2018, 06/14/2018, 06/15/2018, 06/16/2018, 06/18/2018, 06/20/2018, and 06/21/2018. The 11:30 AM dose was administered late on 3 out of 59 dates; 05/20/2018, 06/11/2018 and 06/12/2018. The 4:30 PM dose was administered late on 8 out of 59 dates; 05/07/2018, 05/08/2018, 05/09/2018, 05/13/2018, 05/14/2018, 06/08/2018, 06/11/2018 and 06/20/2018. During surveyor interview on 07/05/18 at 03:06 PM with Staff nurse B, he acknowledged that the medication was being given late and revealed that [MEDICATION NAME]-[MEDICATION NAME] should be given before meals per the physician's orders [REDACTED].> |
