In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

A closed record review for resident ID #24 revealed the resident was discharged from the facility on 1/4/2017 with no evidence of a physician's discharge order. 3. A closed record review for resident ID #29 revealed the resident was discharged from the facility on 7/16/2015 with no evidence of a physician's discharge order. During surveyor interview on 4/7/2017 at 12:30 PM the Director of Nurses unable to provide evidence of a physician's discharge order for resident IDs #23 and 24. During surveyor interview on 4/7/2017 at 10:05 AM the Assistant Director of Nurses was unable to provide evidence of a physician's discharge order for resident ID #29. You are hereby formally notified that where the above listed deficiencies also constitute non-compliance with applicable provisions of the Rules and Regulations for Licensing of Nursing Facilities they are deficiencies under State Regulations and grounds for licensure sanctions. 2020-09-01
