In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: Review of resident ID #4's record revealed a progress note dated 8/22/2018 indicating the following: This evening (Resident ID #4) had multiple verbal behaviors and outbursts and was highly unredirectable. Prior to dinner (s/he) was arguing with another resident and made the statement towards them 'I am going to cut you throat with a knife' . During a surveyor interview with the Administrator on 10/11/2018 at 11:10 AM, she was unaware of the incident and could not provide evidence that the incident was reported to the State Survey Agency. 2020-09-01
