In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: On 6/28/2017 at 12:30 PM, surveyor observation of the first floor failed to reveal evidence of the most recent survey results posted in a public area. During interview with the director of maintenance at this time, he could not locate a posted notice of availability and indicated that the survey results were stored behind the reception desk (where residents do not have free access). Further observation of the second and third floor revealed that survey results were readily accessible, however, the results available were not from the most recent survey. During interview with the Administrator on 6/30/2017, she could not provide evidence of survey results being readily accessible to residents on the first floor or that a notice of their availability was posted. She further acknowledged that the survey results on the second and third floor were not of the most recent survey. 2020-09-01
