In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: Record review for Resident ID #1 revealed s/he was admitted to the facility in 8/2019 with [DIAGNOSES REDACTED]. An admission Minimum Data Set ((MDS) dated [DATE] indicates a Brief Interview for Mental Status (BIMS) score of 15 of 15, indicating the resident is cognitively intact and that the resident requires extensive assistance by two staff members for transfers. Additional record review reveals a Physical Therapy Clarification Order dated 9/10/2019 that states 2 person OOB (out of bed)transfer. Further record review reveals an Individualized Resident Assignment that indicates the resident is an assist of two for transfers. During an interview on 11/07/2019 with Resident ID #1 s/he indicated that s/he had a fall on 11/15/2019. S/he further indicated that the Certified Nursing Assistant (CNA), Staff A transferred him/her independently and did not apply the prosthesis to the left extremity. During an interview on 11/19/2019 at approximately 10:00 AM with Staff A she acknowledged that she attempted to transfer the resident from bed to chair independently without his/her prosthesis on resulting in a fall. 2020-09-01
