In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: Record review of Resident ID# 11's treatment record for (MONTH) 2019, revealed an order to cleanse coccyx wound with normal saline followed by allevyn foam, change every 3 days and as needed. Surveyor observation of Resident ID# 11's dressing change on 10/3/2019 at 11:17 AM by Staff Nurse B, revealed upon completion of the treatment the nurse removed her gloves and exited the room without performing hand hygiene. She then failed to perform hand hygiene prior to entering the nurse's station to document in a resident record. During surveyor interview on 10/3/2019 at 11:23 AM with Staff B, she acknowledged that she did not perform hand hygiene after completing the dressing change. 2020-09-01
