In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: Record review of a 9/11/2018 discharge summary from the Hospital, revealed an order under Discharge Medications for .[MEDICATION NAME] ([MEDICATION NAME])(a medication to treat high blood pressure) 20 mg tablet take 40 mg by mouth daily Record review of the (MONTH) (YEAR) Medication Administration Record, [REDACTED]. This resulted in the resident receiving double the dosage of [MEDICATION NAME] on these days. During a surveyor interview on 10/4/2019 at 10:34 AM with the Director of Nursing Services, she acknowledged that the [MEDICATION NAME] was not administered as instructed on the discharge summary. 2020-09-01
