In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: Record review revealed the facility had a surety bond for $15,000.00 effective 12/21/2018 to 12/21/2019. Further record review of the Resident Trust Fund bank statements revealed: -Account balance on (MONTH) 31, 2019 was $20,534.65 -Account balance on (MONTH) 30, 2019 was $17,614.09. During a surveyor interview with the Business Office Manager on 10/3/2019 at 9:05 AM, she could not provide evidence that the current surety bond covered the resident's funds deposited in the facility for (MONTH) and (MONTH) 2019. 2020-09-01
