In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: Surveyor observations on 3/3/2019 at 12:22 PM, 3/6/2019 at 7:30 AM, and 3/7/2019 at 10:24 AM revealed Resident ID #63 using oxygen via nasal cannula at 2 liters per minute. Review of the clinical record revealed this resident was admitted to the facility on [DATE] with a [DIAGNOSES REDACTED]. There lacked evidence of the attending practitioner's order for oxygen therapy. During a surveyor interview with the Director of Nursing Services on 3/7/2019 at 12:23 PM, she revealed that the resident should have a practitioner's order for the oxygen. 2020-09-01
