Findings are as follows: Surveyor observations on 3/3/2019 at 12:22 PM, 3/6/2019 at 7:30 AM, and 3/7/2019 at 10:24 AM revealed Resident ID #63 using oxygen via nasal cannula at 2 liters per minute. Review of the clinical record revealed this resident was admitted to the facility on [DATE] with a [DIAGNOSES REDACTED]. There lacked evidence of the attending practitioner's order for oxygen therapy.
