In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: 1. Record review revealed that Resident ID #29 was admitted to the facility on [DATE]. The resident was admitted to Hospice services on 8/23/2018. Further record review revealed that a Significant Change in Status Assessment was not completed when the resident was admitted to Hospice services. 2. Record review revealed that Resident ID #31 was admitted to the facility on [DATE]. The resident was discharged from Hospice services on 1/11/2019. Further record review revealed that a Significant Change in Status Assessment was not completed when the resident was discharged from Hospice services. During a surveyor interview with the Minimum Data Set nurse, Staff A, on 3/7/2019 at 10:47 AM, she could not provide evidence that a Significant Change in Status Assessment was completed for either resident when they had a change in Hospice services. 2020-09-01
