In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: Record review for Resident ID# 45 revealed the following: 1. A current physician's orders [REDACTED]. 2. A current physician's orders [REDACTED]. 3. A current physician's orders [REDACTED]. Surveyor observation during the Medication Administration Task on 1/3/2019 at 8:16 AM revealed the Unit Nurse (Staff B) leaving a medication cup with the [MEDICATION NAME] and [MEDICATION NAME] on the resident's bedside table while the resident was in the bathroom. During surveyor on 1/3/2019, immediately following the observation, Staff B revealed that the resident self-administers the medications and the [MEDICATION NAME] is stored in the bathroom, not in a locked compartment. Additionally, she has been leaving the resident's medications on the bedside table for a while and acknowledged that they were not secured. During a subsequent interview on 1/3/2019 at 9:35 AM, the Administrator was unable to provide evidence that the resident's drugs were stored in locked compartments. Additionally, during interview on 1/4/2019 at 8:19 AM, Resident ID# 45 confirmed that the [MEDICATION NAME] has been stored in the bathroom and that the nurses leave medications on the bedside table. 2020-09-01
