In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: Record review for Resident ID# 42 revealed an admission date of [DATE] with [DIAGNOSES REDACTED]. Further review of the record revealed the following: 1. A 12/5/2018 physician's orders [REDACTED]. Review of the Medication Administration Record [REDACTED]. 2. A 12/20/2018 physician's orders [REDACTED]. Review of the MAR indicated [REDACTED]. 3. A 12/26/2018 physician's orders [REDACTED]. Review of the MAR indicated [REDACTED].Other for 3 of 9 opportunities. Surveyor observation during the Medication Administration Task on 1/3/2019 at 7:31 AM revealed that the current physician ordered B Complex 1 dose was not available to the resident. During an surveyor interview, immediately following this observation, Medication Aide (Staff A) revealed that the resident did not receive the B Complex and that the medication has not been available for a few days. During a subsequent interview on 1/3/2019 at 10:26 AM, the South Wing Charge Nurse was unable to provide evidence that the above physician ordered medications were available to Resident ID# 42. 2020-09-01
