In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This placed residents at risk for lack of care coordination related to [MEDICAL TREATMENT] treatment. Findings include: Resident 66 was admitted to the facility in 6/2016 with [DIAGNOSES REDACTED]. A 6/9/16 physician's orders [REDACTED]. The facility's Policy & Procedure for [MEDICAL TREATMENT] Agreement document directed, (the facility) will have the [MEDICAL TREATMENT] center sign the attached agreement which clarifies each providers' responsibilities in regards to the [MEDICAL TREATMENT] and access site. On 8/11/16 at 12:11 pm Staff 2 (DNS) was asked for a copy of Resident 66's [MEDICAL TREATMENT] agreement. Staff 2 stated it should be in the resident's medical record. A review of the resident's medical record did not reveal a [MEDICAL TREATMENT] agreement. On 8/11/16 at 1:48 pm Staff 7 (RNCM) stated she could not locate a completed [MEDICAL TREATMENT] agreement for Resident 66. 2020-09-01
