In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This placed the resident at risk for unsafe medication administration. Findings include: Resident 285 was admitted to the facility in 8/2017 with [DIAGNOSES REDACTED]. On 9/5/17 at 9:27 am Resident 285 was observed lying in bed. A plastic cup containing three pills was observed on the resident's overbed table. On 9/5/17 at 9:31 am Staff 10 (CMA) stated she left the pills with Resident 285 at the resident's request and did so often. Staff 10 stated the pills included two cranberry tablets and a multivitamin. Review of Resident 285's record did not indicate Resident 285 was assessed to administer medications on her/his own. On 9/5/17 at 11:40 am Staff 2 (DNS) acknowledged Resident 285 was not assessed to self-administer the medications. 2020-09-01
