Findings include: Resident #2 Resident #2 was admitted on [DATE], with [DIAGNOSES REDACTED]. The Resident Authorization Consent Form dated 2/19/18, was signed by the resident identifying the resident's family member as the authorized representative to receive information regarding the resident's condition, scheduled appointments and/or meetings regarding resident's care. The Resident Transfer Form dated 2/21/18, reflected a transfer to a local hospital for [DIAGNOSES REDACTED]. The Document entitled Discharge census listed the resident as being transferred to a local hospital on [DATE] at 4:25 PM. On 3/15/18 at 1:36 PM, a Social Worker explained the facility's process was to contact the next of kin or authorized representative when a resident was transferred to the hospital and staff would be document in nurse's notes. On 3/15/18 at 1:45 PM, a Licensed Practical Nurse (LPN) explained the facility process was staff would inform next of kin or authorized representative in an event of a transfer to a hospital. The LPN explained this was documented in the nurse's notes. The nursing notes lacked documented evidence the family member was notified of the hospital transfer. On 3/15/18 at 3:45 PM, the Director of Nursing (DON) verified the family was not notified of the resident's transfer to the hospital. | 2020-09-01 |
