The findings are: A: On 10/28/18 record review of the facility Infection Control Tracking log revealed that the log only contained documentation starting in (MONTH) for the current year (08/18). B: On 10/28/18 at 12:40 pm, during an interview Director of Nursing (DON) stated, It looks like it starts in (MONTH) (08/18), I don't have any information prior to that.
