In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This deficient practice could likely result in residents not receiving the assistance needed to accomplish everyday tasks, resulting in a decline of in function and depression from not being able to assist themselves. The finding are: [NAME] Record review of the MDS assessment dated [DATE] revealed the following: Eating: self-performance 1 (Supervision - oversight, encouragement or cueing). B. Record review of the MDS assessment dated [DATE] revealed the following: Eating: self-performance 2 (Limited assistance - resident highly involved in activity; staff provide guided maneuvering of limbs or other non-weight-bearing assistance). C. On 09/20/17 at 9:26 am, during an interview, the MDS Coordinator (MDSC #1) stated, If we (staff) open anything (drinks food items) for them (residents) then it is coded at 2 limited assistance. MDSC also stated, She (R #26) is limited assistance because staff have to help her open containers and cut her food. D. On 09/20/17 at 10:50 am, during an interview CNA #6 stated that R #26 eats in her room and does need staff to add salt and open containers for her. Also, CNA #6 confirmed that R #26 had needed this assistance since she started to work with her. E. On 09/20/17 at 9:55 am, during an interview and record review the Human Resources Director revealed CNA #6 had been working at the facility since 11/22/16. F.
