In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on record review and interview, the facility failed notify the resident of the facility's bed hold policy for 1 of 1 transferred residents in a sample of 22 residents. (Resident identifier #55) Findings include: Review of Resident #55's progress notes on 4/13/18 at 08:56 am revealed Resident #55 was transferred to the hospital on [DATE]. Further review of the resident's medical record revealed [REDACTED]. Interview with Staff I (Regional Director of Operations) on 4/13/18 03:02 pm confirmed that there was no written evidence of notification of the bed hold policy for Resident #55 on 4/9/18. Interview further revealed that it is the facility notifies residents of the facility's bed hold policy on admission and not at transfer. |
