In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The facility staff identified a census of 128. Findings are: [NAME] Record review of a Physician Order's Sheet for 6-2017 revealed an order for [REDACTED]. Record review of Resident 219's Medication Administration Record [REDACTED]. Review of Resident 219's medical record that included progress notes, physician progress notes [REDACTED]. blood pressures as directed. On 6-6-2017 at 10:41 am an interview was conducted with the facility Director of Nursing (DON). During the interview the DON reported not being able to locate any information that Resident 219's physician had been called when Resident 219's blood pressures were low as directed. |
