In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: During an observation on 11/13/18 at 12:45 p.m., the Daily Posting of Hours of Nurse Staffing sheet, posted near the nurse's station, had not been updated since 11/5/18. The posted information sheet also lacked the facility's name. During an interview on 11/13/18 at 4:06 p.m., staff member A stated she was not sure when, or by whom, the Daily Posting of Hours for nurse staffing was completed. A review of the Daily Posting of Hours showed the sheets had not been updated from 10/16/18 through 10/24/18, and 10/26/18 through 11/3/18. The posted information sheet lacked the facility's name. During an observation on 11/14/18 at 4:44 p.m., the Daily Posting of Hours had not been updated since 11/13/18. The posted information sheet lacked the facility's name. During an observation and interview on 11/15/18 at 1:15 p.m., staff member [NAME] stated the Daily Posting of Hours had not been updated since 11/13/18. Staff member [NAME] stated, The Night-shift (nurse) was responsible for initiating the Daily Posting of Hours, but that nobody was currently responsible for making sure it had been completed. 2020-09-01
