Findings include: During an observation on 4/16/19 at 6:23 p.m., resident #17 received pureed soup and ice cream for dinner, and the resident was not served the entire pureed meal, per the facility menu plan. Review of resident #17's Nutrition Care Plan, dated 4/11/18, showed she had the potential for alteration in nutrition. The care plan was not updated with a severe weight loss, and the plan did not specify the resident's dinner meal should be limited. Review of the nutrition goal showed, I will not lose greater than five percent (weight) and I will eat greater than 75%. All interventions, except for one, were dated 4/11/18. A new intervention, dated 11/15/18, was for a mechanical soft diet texture. A review of resident #17's physician orders [REDACTED]. The resident's weight loss had been occurring over the past year. During an interview on 4/17/19 at 8:20 a.m., staff member T stated she would update the resident's care plan with any new interventions. 2020-09-01
