In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This was for one (1) of 15 care plan reviewed. Findings Include: Review of the facility policy titled, Nursing Care Plan, stated, Customize the care plan to address the Resident's individual concerns and needs. Review of the resident's care plan revealed the absence of a care plan to address the wound care to the area on the left buttock. Record review revealed a physician's order dated 10/28/16 that stated, Cleanse abrasion to left buttock with normal saline, apply duoderm every three days. Interview with Registered Nurse (RN) #1 on 04/19/17 at 11:10 AM confirmed that she had not implemented a care plan for the skin concern area to the left buttock that developed on 10/28/16. RN #1 also confirmed that it was her responsibility to develop a care plan when the wound care began on Resident #8 and stated that she just failed to do it. Resident #8 was admitted to the facility on [DATE] with a [DIAGNOSES REDACTED]. status was severely impaired and she was unable to complete the assessment. 2020-09-01
