In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings Include: Review of the facility's policy, titled, Labeling Resident Items, dated 2/22/16 revealed, Purpose: Identify items for personal use to avoid sharing of personal items. Maintain personal items labeled for resident use. During the initial environmental tour on B Hall on 2/22/16 with Licensed Practical Nurse (LPN) #1 at 10:30 AM to 11:45 AM, the following shared bathrooms contained personal items that were unlabeled and not bagged. Room B1 shared bathroom contained all unlabeled: dishwashing detergent, hand sanitizer, mouthwash, two tubes of toothpaste, two toothbrushes, and a hairbrush on the lavatory. Room B3 shared bathroom contained an unlabeled toothbrush and drinking cup. Room B6 shared bathroom contained an unlabeled toothbrush and toothpaste. Room B10 shared bathroom contained all unlabeled: bottle of shampoo, shaving cream, urinal, perineal cleanser, and toothbrush. Room B16 shared bathroom contained an unlabeled urinal. Interview with LPN #1 on 02/22/16 at 10:30 AM revealed these items should not be in these bathrooms. LPN #1 stated these bathrooms are shared by two residents and we need to get the personal items out. Interview with the Administrator on 2/23/16 at 10:00 AM stated staff have been trained to label all personal items when placed in resident rooms. Stated personal items should not be shared. 2020-09-01
