In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Resident #4 was not taking [MEDICAL CONDITION] medication upon admission. During an Interview on 10/31/2108 at 10:50 AM, Licensed Practical Nurse (LPN) #1 stated that she's responsible for the PASSR. LPN #1 stated that all newly diagnosed residents with mental or intellectual disorders should be sent to the appropriate agency (Agency Named) to decide whether a level 2 should be done, however, she was unable to get to it at this time. She stated she will take care of the PASSR for Resident #4 as soon as possible (ASAP). LPN #1 stated that the facility does not have a policy on PASSRs, but it is part of her job duty to make sure they are done correctly and timely.
