In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings: Instructions on the PASARR Level I Screen, Terminal Illness Waiver, states, Should the individual's status change, a new Level I Screen indicating a change in condition must be submitted to KEPRO for review. During review of Resident # 19's clinical record, the surveyor noted that Resident #19 has a [DIAGNOSES REDACTED]. The Level I PASARR dated 4/18/18 had a Terminal Illness Waiver which states, Should the individual's status change a new Level I Screen indicating a change in condition must be submitted to KEPRO for review. On 9/26/18 the Hospice Agency issued a discharge revocation of services. On 12/12/18 at 03:15PM in an interview with the Social Worker, the Surveyor confirmed with the Social Worker that currently there is no evidence that a new PASARR Level I was completed following the change in condition per Terminal illness Waiver instructions. 2020-09-01
