Finding: On review of Resident #55's clinical record, the surveyor noted the resident's current physician's orders [REDACTED]. The Pharmacist's Notes to Attending Physician/Prescriber, dated 5/2/17 and 6/2/17, did not address nor recommend a gradual dose reduction for the antipsychotic. The surveyor also reviewed the Drug Regimen Review from 5/2/17 through 10/2/17, and could not determine any recommendations made for a gradual dose reduction. In an interview with the surveyor on 10/26/17 at 12:09 p.m., the Interim Administration confirmed there is no evidence of an attempted gradual dose reduction of the antipsychotic Risperidone. | 2020-09-01 |
