In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This was true for 1 of 2 medication storage rooms with expired Pneumococcal vaccine. This failed practice had the potential to effect 12 of 12 sampled residents who could receive expired medications (#2, #3, #4, #7, #8, #9, #16, #19, #20, #29, #30, and #132) and other 23 residents who resided in the facility. This failed practice created the potential for harm should residents receive expired vaccinations with decreased efficiency. Findings include: The facility Medication Management policy dated 11/28/17, documented medications were discarded by the expiration date unless indicated by the pharmacy and/or the manufacturer's instructions to discard sooner. On 6/14/18 at 10:58 AM, a medication room was inspected with RN #4. An unopened multi-dose vial of Pneumococcal vaccine with an expiration date of 9/27/17, was found in the refrigerator. At the time of inspection, RN #4 verified the expiration date and disposed of the expired medication. On 6/14/18 at 11:08 AM, the DON stated it was the staff and administration's responsibility to monitor for expired medications. Residents #2, #3, #4, #7, #8, #9, #16, #19, #20, #29, #30, and #132 and the other 23 residents residing in the facility were at risk of receiving expired Pneumococcal vaccine. 2020-09-01
