In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This had the potential to affect 7 of 7 (#s 1-7) sampled residents and all residents who dined in the facility. This failure created the possibility for food-born illness if bacteria remained on unsanitary surfaces. Findings include: On 7/26/17 at 11:05 am, 7 of 11 plastic coffee mugs were observed to contain a white filmy residue inside the mugs. The Lead Food Service Worker (LFSW) said it looked and smelled like soap residue. The LFSW said he would have them rewashed and wiped clean. 4 of 6 plastic soup and cereal bowls were observed to have several scratches inside the bowls. The LFSW said they all had scratches and grooves in them and he took them out of service and said he would replace them with new bowls. 2020-09-01
