In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

There were raised toilet seats in disrepair in 2 (B5, F6) out of 36 resident rooms reviewed. Findings include: 1. On 10/15/18 at 3:04 PM and on 10/23/18 at 1:51 PM, the raised toilet seat in the bathroom of room B5 was observed with peeling paint and having rust. 2. On 10/15/18 at 3:58 PM and on 10/23/18 at 2:01 PM the raised toilet seat in the bathroom of room F6 was observed with peeling paint and rust.
