In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

> Based on observation and interview, it was determined that the facility failed to ensure proper infection control techniques during medication administration for two (R39 and R72) out of 55 Stage 2 sampled residents. Findings include: 1. During medication administration for R39 on 7/13/17 at 9:50 AM, E8 (LPN) was observed touching the trash can lid on the medication cart when throwing out trash and then continuing to touch medications and the medication cart without hand sanitizing or washing his/her hands. 2. During medication administration for R72 on 7/13/17 at 1:45 PM, E8 was observed touching the trash can lid on the medication cart when throwing out trash and then continuing to touch medications and the medication cart without hand sanitizing or washing his/her hands. During an interview with E8 on 7/18/17 at 1:50 PM, the findings were reviewed and confirmed. The findings were reviewed with E2 (DON) and E3 (RN,Staff Development) on 7/19/17 at 2:45 PM. |
