In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: 1. Review of R197's clinical record revealed the following: 3/10/17 - A physician's orders [REDACTED]. 3/10/17 - A physician's orders [REDACTED]. R197's progress notes lacked evidence of her behaviors and non-pharmacological interventions used prior to the prn [MEDICATION NAME] administrations on the following dates and times: - 3/13/17 at 6:35 PM; - 3/15/17 at 2:01 PM; - 3/20/17 at 5:53 PM; - 3/21/17 at 9:33 PM; - 4/3/17 at 7:30 PM; - 4/19/17 at 6:58 PM; - 6/14/17 at 1:18 PM; - 6/26/17 at 3:21 PM; - 7/10/17 at 3:23 PM. 2. Review of R143's clinical record revealed the following: 2/11/16 - A physician's orders [REDACTED]. 1/31/17 - A physician's orders [REDACTED]. R143's progress notes lacked evidence of her behaviors and non-pharmacological interventions used prior to the prn [MEDICATION NAME] administrations on the following dates and times: - 7/1/17 at 2:52 PM; - 7/4/17 at 4:57 PM; - 7/8/17 at 2:10 PM; - 7/10/17 at 6:48 PM. During an interview on 7/19/17 at 1 PM, E13 (RN) acknowledged that non-pharmacological interventions should be used prior to the administration of prn [MEDICAL CONDITION] medications. Findings for R197 and R143 were immediately reviewed with E13. Findings were reviewed with E2 (DON) and E3 (RN/Staff Ed) on 7/19/17 at 3 PM.
