R8 fell on [DATE], was sent to the ER (emergency room ) and received stitches for a cut in her eyebrow. Findings include: 1. Review of R8's clinical record revealed the following: R8 resides in the locked dementia unit of the facility. She has both short and long term memory problems according to a 7/2/17 annual MDS. R8 fell on [DATE] and was sent to the ER where she received 3 stitches to her right eyebrow. While reviewing the facility's investigation of the fall, there was no documentation that the facility reported the fall to the DLTCRP State agency within 24 hours and no documentation of a 5 day follow up being sent to the DLTCRP. During an interview with E2 (DON) on 9/7/17 at 11:20 AM, E2 confirmed that R8's 1/3/17 fall with injury that required the resident to be sent to the hospital and treated was not reported to the DLTCRP. Findings were reviewed with E2 (DON) and E4 (ADON) on 9/7/17 at approximately 3: 30 PM. | 2020-09-01 |
