In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Based upon observations and interviews, it was determined that for 2 (R10 and R11) out of 11 sampled residents, the facility failed to protect their privacy and confidentiality of their medical records. Findings include: 1. On 4/24/18 at 11:22 AM in the G Wing hallway, the surveyor observed R10's eMAR displayed on the computer screen of G medication cart unattended. E26 (LPN) exited a resident's room and returned to the unattended medication cart. E26 stated that she left her medication cart to respond to a resident calling for help. 2. On 4/24/18 at 5:05 PM in the F Wing hallway, the surveyor observed R11's eMAR displayed on the computer screen of F medication cart unattended. AE4 (LPN) exited a resident's room and returned to the unattended medication cart. AE4 stated that she left her medication cart to assist a resident with toileting. Findings were reviewed with E3 (Staff Educator) on 4/24/18 at 5:15 PM. The facility failed to protect the privacy and confidentiality of R10 and R11's medical records.
