In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on observation, record review and interview, it was determined that for one ( R27) out of 23 sampled residents, the facility failed to ensure the resident's drug regimen was free from unnecessary drugs. Findings include: 1. Review of R27's clinical record revealed: R27 was admitted to the facility on [DATE] with [DIAGNOSES REDACTED]. R27 was prescribed an anti-psychotic medication. R27's documentation survey report indicated she was to be monitored for behaviors every shift. Between (MONTH) 23 ,2018 and (MONTH) 15, (YEAR) there were 223 opportunities to monitor R27's behavior. Only 193 opportunities were noted on the behavior documentation survey report, with 30 shifts left blank. There was no evidence that the facility consistently monitored R27's behaviors. Findings were reviewed with E2 (DON) and E3 (ADON) on 7/18/18 at approximately 2:00 PM. |
