On (MONTH) 19, (YEAR), review of the Treatment Administration Record (TAR) dated (MONTH) 1, (YEAR), revealed, a d/c (discontinue) foley catheter as a prescribed order. The clinical record lacked documented evidence that the physician wrote an order to discontinue the foley catheter. During a telephone interview with Employee #14 on (MONTH) 1, (YEAR), at 11:00 AM, the employee was questioned about the foley catheter order.
