In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The findings include: One (1) of one (1) dishwashing machine failed to reach and maintain a final rinse temperature of 180 degrees Fahrenheit on (MONTH) 30, (YEAR), at approximately 10:40 AM. During several, consecutive wash cycles, the final rinse temperature gauge was at or below 164 degrees Fahrenheit. A stack pump was added to the dishwashing machine by Ecolab at approximately 11:45 AM on (MONTH) 30, (YEAR), to circumvent low final rinse temperature issues and to enable the facility to use chlorine as a disinfectant. Dishes disinfection occurred after test strips confirmed that the disinfectant solution was at a minimum of 50 Parts per Million (PPM). The observations made in the presence of Employee #22 were acknowledged. 2020-09-01
