Further review revealed a Minimum Data Set assessment dated (MONTH) 16, (YEAR). The assessment reveals the facility staff documented the number zero (0) in Section B1000 (Ability to see in adequate light with glasses or other visual appliances). This coding indicates the resident has Adequate Vision; sees fine detail such as regular print in newspapers/books. Further review of the Minimum Data Set assessment dated (MONTH) 16. (YEAR), Section I (Active Diagnoses) reveal the Resident ' s additional active [DIAGNOSES REDACTED]. The medical record lacked documented evidence that the MDS coding accurately reflects the resident's condition; [MEDICAL CONDITION].
