In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: A dining observation was conducted on (MONTH) 3, (YEAR) at 11:45 a.m. on the Kalmanovitz secured dementia unit. During this observation, a CNA (Certified Nursing Assistant/staff #185) was observed to touch a resident's muffin, with her bare hands. An interview was conducted on (MONTH) 4, (YEAR) at 12:30 p.m., with the CN[NAME] Although the CNA was unable to recall if she had touched any resident's food with her bare hands, she stated that would be a sanitary problem. Another interview was conducted on (MONTH) 4, (YEAR) at 2:30 p.m., with the Director of Nursing (staff #161) who stated that staff should not handle ready to eat food with their hands and that utensils should have been used. A facility policy titled, Assistance with Meals included: 7. All employees who provide resident assistance with meals will be trained and shall demonstrate competency in the prevention of foodborne illness, including personal hygiene practices and safe food handling. 2020-09-01
