In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This affected Resident Indentifer (RI) #8, one of eight sampled residents whose meals were observed on 3/15/17 and had the potential to effect 40 of 50 residents receiving meals from the kitchen. Findings Include: Resident Identifier (RI) #8 was readmitted to the facility on [DATE], with [DIAGNOSES REDACTED]. A review of a facility document titled MENU w/DIETS for 3/15/17 documented .noon day 18 .ALTERNATE . REGULAR .1/2 c (cup) Roasted Vegetables .MECHANICAL SOFT .Roasted Vegetables . On 3-15-17 at 11:45 a.m., the surveyor observed kitchen staff plating food. When plating the roasted vegetables the staff used kitchen tongs to serve the Roasted Vegetables. On 3-15-17 at 12:11 p.m., during the lunch meal, RI # 8's was served roasted vegetables. On 3-15-17 at 5:00 p.m., an interview was conducted with Employee Identifier (EI) # 8. EI # 8 was asked about the roasted vegetables being served with tongs during lunch on 3-15-17 when the menu called for 1/2 cup. EI #8 replied the staff should have used a scoop or ladle and it couldn't be measured accurately using tongs. 2020-09-01
