In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This filed practice had the potential to effect all residents, based on a census of 21, to receive less than optimal care. Findings: Document review on 8/23/19 at 10:14 am, of the Community Health Board minutes, identified by the Administrator (AD) as the Governing Body (GB), revealed no information on reporting, policies, management or operation of the long term care (LTC) facility for the past 12 months. The content of the minutes was strictly related to hospital governance. During an interview on 8/23/19 at 10:16 am, the AD stated there was no LTC representative who reported information to the GB. The AD further revealed there had been no reports submitted to the GB. The AD had tried to get on the agenda and had been cancelled. The AD stated there is no GB oversight for the LTC. 2020-09-01
